Job Overview
This is an exciting opportunity for a pharmacist to take a leading role in the management of long-term conditions, helping to improve patient outcomes while working closely with GPs and the wider practice team. The successful applicant will lead and support the management of patients with long-term conditions including:
- Asthma and COPD
- Type 2 Diabetes
- Hypertension
- Chronic Kidney Disease (CKD)
- Cardiovascular disease prevention and lipid management
Conduct structured medication reviews and optimise prescribing. Monitor, review, and manage patients in accordance with local and national guidelines. Contribute to quality improvement initiatives and chronic disease recall systems. Develop personalised care plans and support patient self-management. Work collaboratively with GPs and the wider multidisciplinary team to deliver high quality, evidence-based care. Full clinical support will be provided to interpret clinical investigations and laboratory results to inform treatment decisions.
Main Duties of the Job
- Provide independent prescribing within agreed clinical areas
- Manage acute prescribing requests
- Identify and resolve medicines-related safety issues
- Contribute to medicines optimisation and prescribing governance activities
- Support safe, effective, and cost-efficient prescribing
Person Specification
Essential Criteria
- Registration with the General Pharmaceutical Council (GPhC)
- Independent Prescribing qualification
- Strong clinical knowledge and prescribing confidence
- Patient-focused approach with commitment to high-quality care
- Ability to work effectively within a multidisciplinary team
Desirable Criteria
- Experience in managing Long term conditions in Primary Care: respiratory disease, diabetes, cardiovascular risk management, HRT or CKD.''
